To make a Child protection referral, please send all information to the Initial Response Service (The Quest) on a Common Assessment Framework (CAF) form.
If you are using this as a Child Protection referral from, please put as much information on to pages 1-7 as possible. This form must be used to support all verbal referrals and be sent within 24 hours. Please note that Walsall practitioners are now required to use the new CAF Form which has some differences to the old version. If you would like support in filling it out, or would like to clarify any sections please contact us on 01922 650497. See also guide to definitions used in the Common Assessment Framework form
