Serious Case Review Panel


Serious Case Reviews (SCR) are undertaken when a child dies and abuse or neglect is known or suspected to be involved; or a child is seriously harmed and there is cause for concern as to the way in which agencies worked together to safeguard the child.

All relevant staff should be aware of when Serious Case Reviews are required or should be considered. Each SCR will have its own specific terms of reference defined.

  • Organisational and management reviews are commissioned.
  • An independent person compiles an overview report.
  • The report is received and endorsed by the SCR panel.
  • An action plan and recommendations are agreed.
  • The Operations Group will monitor compliance with the action plan.
  • Learning from a SCR should be disseminated in an appropriate manner.
